What type of insurance do you participate in?

Falls Medical Specialists, at our Greenspring office, participates with many major insurance carriers, including:

NOTE: It is the patient’s responsibility to obtain any referrals needed at the time of each visit.

 

What are the laboratory hours?

Monday–Friday,  8:30 – 4:00. We close for lunch everyday from 12:30 – 1:00.

 

What are the infusion clinic hours?

Monday – Friday, 8:30 – 4:30.

 

What is your cancellation policy and fees?

If you are more than 10 minutes late for an appointment, you will be marked a No Show. Failure to arrive on time for your appointment will result in a $50 fee. 24 hours notice is required to cancel and/or reschedule all appointments. Failure to do so will result in a $50 No Show fee.

 

Are there additional fees for work forms, FMLA forms, etc?

There is no additional charge if done at the time of visit. Otherwise, there is a $30 form fee.

 

What is required before I make my appointment with internal medicine or rheumatology? Do I need to have records sent?

We request that our new rheumatology patients have medical records faxed to our office prior to their initial visit. These records would include: laboratory reports, radiology reports to include MRI or CT studies, as well as physician notes from any consultations related to your rheumatologic issues. What do I have to bring with me to my appointments?

  • Health Insurance card(s)
  • Drivers License or other form of picture ID
  • Method of Payment

 

How do I obtain medical records?

In most cases, you can request your records directly from your doctor. Keep in mind that offices may only keep records for a certain amount of time as required by the individual state. You should call the office to make sure your records are still on file. All patients must sign a consent form before our records are released.
